Mudbath Player attendance and Parent volunters

All Boys and Girls players must fill this out with a parent or guardian by 4/19. This tournament is the best way for players to improve and bond with their teammates along with having a lot of fun! This is free for players and is our main source of funding for Memorial Ultimate. We do need parent/guardian support to help the tournament run smoothly. This is done by parents/guardians volunteering for 3-4 hours at tournament central during the weekend to help sell merchandise/discs, direct players to the correct fields, and make sure water is availible to everyone.

USAU Number Submission Form

ALL PLAYERS Deadline of 3/29 for submission or talk to a coach!
A USAU membership is necessary to be rostered on the team and compete in games. See more info under the How to Join tab.

Medical Form

Please print and hand in to a coach. You will need this to play in the first game! Don't worry about who the chaparones are as we just need to know who to contact if something were to happen.

GroupMe

Players and parents should join the GroupMe chats for quick communication on real-time game and practice information. You will need to be approved to join as this is a public link, please allow us one day to do this.
